o The applicant is to complete and provide the " Checklist for Local ~f <br />Government to Determine Whether a Facility is Categorically Excluded". <br />The form is to be signed by an authorized representative of the applicant. <br />• This basically excludes the facility if the antenna (omni-directional or <br />directional of one sector is 10 meters (33 ft.) AGL and total effective <br />ERP of all channels is 1000 (cellular) or 2000 (PCS) watts or less. <br />c The FCC's limits apply cumulatively to all sources of RF emissions affecting <br />a given area. Therefore, the data/information utilized in the determination <br />must be cumulative (include all antenna installations on or proposed on the <br />facility). <br />• Not Categorically Excluded - If the site is not categorically excluded and/or on <br />visual siting of the proposed multiple-use co-location site and/or rooftop proposal it <br />is determined that there is major concern by the community and/or consultant as to <br />the site being in full compliance than the applicant will be required to provide a full <br />cumulative study with all data to verify <br />o The full report with data is to be as per guidelines of Uriel Oko, Ph.D. P. E. <br />dated 3/26/03 <br />o If we determine from the study that the facility exceeds (generally a rooftop <br />installation) what is required to be in compliance, the applicant must agree <br />to construct the site with appropriate measures (and provide specifics) such <br />as barricades to bring the site in compliance as to RF exposure to humans. <br />9. Attendance at Site Visit and Pre-Application Meeting: It is strongly recommended that the <br />applicant have the person in charge of preparing the application at the site visit and the pre- <br />application meeting, as well as the person(s) who will be responsible for the production of <br />Propagation Studies and NIER (RF emissions) information. This is intended to save the <br />applicant time and money by helping to assure that the application contains all the <br />information that is needed to make a determination, in the format needed for the most <br />efficient review of the information, and prevent having to incur delays and/or increased <br />costs review costs caused by having to subsequently obtain the needed and correct <br />information. <br />10. Engineer's Certification: where required, a Professional Engineer licensed in the state must <br />affix a Professional Engineer's stamp & certification. <br />11.
