A filing fee must accompany all applications. The fee varies depending upon the type of application and is adjusted
periodically by the City Manager. Check with Planning staff at the Permit and Information Center to determine the
required fee or check website at www.eugeneplanning.org

Final plat mylars
Concurrences
Performance Bond, if required for public improvements.
Documentation of water service availability to each lot in the plat.
All documents specified as Draft in Stage 1 shall be final.
Additional documentation or evidence to demonstrate compliance with tentative approval conditions and final plat
approval criteria, as required.
Post-monumentation certificate, identification of corners to be post-monumented, deposit estimate, and check, as
required.

Submittal Requirements:
Provide 2 paper copies and one digital copy (CD, USB Drive, or Other) of all application materials (i.e. written statement,
site plans, etc.). Digital files must be in PDF file format. Please note that it is the applicant’s responsibility to make sure
that the digital and paper copies are identical. Following completeness review, an updated digital and paper copies may
be required. All paper site plans must be folded to a size equal or less than 11” x 17”.
Written statement that indicates how the final application addresses the final plat approval criteria and the conditions of
approval (informational items identified during the tentative approval may be applicable during the final plat stage).
Plat signed by an Oregon licensed land surveyor. A list of plat requirements subject to City Survey staff review is
available at the City of Eugene Public Works offices, at 858 Pearl Street, Eugene, OR 97401.
As required, include the following:
Delineation of the flood plain or floodway boundaries.
The community number, map/panel number and effective date of the relevant FIRM map.
A note including the Base Flood Elevation for each lot.
A stamped, dated narrative from the surveyor or engineer explaining the methodology used to determine the
Base Flood Elevation.
The applicant is responsible for submitting a copy of the plat signed by an Oregon licensed land surveyor to the Lane
County Surveyor’s office along with necessary review fee.
A current title report issued by a title insurance company licensed by the State of Oregon, verifying ownership by the
applicant of the real property.
Closures for lots, right-of-way, centerline, and irregularly shaped easements.
Draft Irrevocable Petition for Public Improvements, as required
Draft Bargain and Sale Deeds for reserve strips, as required.
Draft Conditions, Covenants, and Restrictions or Joint Access Easement and Maintenance Agreement, as required
Drywell certification signed by a civil engineer, based on field-observed soil types and percolation rates, as required.
Draft copies of off-site easements and right-of-way, as required.
Draft storm drainage maintenance agreement, as required.
Copy of the receipt for the Street Tree Agreement submitted to the City’s Urban Forester, as required.
Copy of wetland documents, as required.
Deed restrictions, as required.
Submit a legal description of property included in the subdivision application. This legal description must be typed on an
8½" x 11" white sheet of paper (no letterhead) so that it is suitable for recording.
Alternate Methods and Materials (AMM) application, fire separation easement and receipt demonstrating the AMM has
been submitted to Building and Permit Services staff for review, as required.
NOTICE STATEMENT: If the Eugene/Springfield Fire Marshall (or the Fire Marshal’s designee) determine that there is
inadequate water supply, apparatus access, or both, to the site for the development of one and two family dwellings
that will be subject to the Oregon Residential Specialty Code, the Eugene Building Official (or the Building Official’s
designee) may require compliance with one or more of the fire suppression or fire containment Uniform Alternate
Construction Standards set out at OAR 918-480-0125(4) and (5).

DECLARATION OF A PRIVATE ROAD, PRIVATE UTILITY EASEMENT, PUBLIC
EMERGENCY VEHICLE EASEMENT AND MAINTENANCE AGREEMENT
KNOW ALL MEN BY THESE PRESENTS, that Willamette Christian Center is the owner of property platted as
Juniper Springs in Lane County, Oregon.
FURTHER, said Owners create a Private Street, Emergency Access and Utility Easement shown as Private Road on
the Juniper Springs plat.
NOW THEREFORE, it is hereby agreed and declared that:
1. The Owners, Heirs, Assigns and Successors of Lots 18 through 45 of Juniper Springs Plat shall bear all
cost of installation of improvements within said Easement. All improvements approved as part of the
land use decision identified as ST 25-03, Juniper Springs are allowed to be constructed in this
easement.
2. Said easement is also a public emergency vehicle access easement in perpetuity.
3. The Owners, Heirs, Assigns and Successors of Lots 18 through 45 of Juniper Springs Plat shall cause
said Private Road and public emergency vehicle access easement to remain free of structures.
4. The Owners, Heirs, Assigns and Successors of said Lots 18 through 45 of Juniper Springs Plat shall
share the costs of maintenance of joint driveway within said Easement.

Conditions of Approval
1. The applicant shall provide an updated calculation of areas to be excluded for
public and private streets and alleys, public parks, and other public facilities to
demonstrate
compliance with the net density requirement at EC 9. 2751( 1).
The net development area on the site minus right of way, private street and public storm
tract is 9.77 acres. There are 57 proposed platted lots that equates to a net density of
5.83 units per acre. Therefore this condition is met.
2. The easternmost north -south portion of the private street shall be relocated to
be east of Lots 33 and 36 and west of Lots 34 and 35, to result in no lots having
frontage on two non -intersecting streets in compliance with EC 9. 2761. The
applicant shall demonstrate that any reconfigured lots continue to meet the
requirements of EC 9. 2760, EC 9. 6505, EC 9. 6735, and EC 7. 420. The
reconfigured private street and impacted lots shall be shown on the final plat and
included on the final site plans.
The easternmost North-South portion of the private street located in Tract D has been
moved to the West of lots 34 and 35 so there are no double frontage lots in the
development. The final plat map shows the reconfigured lots 34, 35, 42-45 meet all of
the requirements in Eugene Code sections. Therefore, this condition has been met.
3. Prior to final site plan approval, the applicant shall submit an updated site plan
that depicts the construction of the temporary emergency vehicle turnaround
shown encumbering Lots 12- 15. The emergency vehicle turnaround shall be
designed to be consistent with the Public Improvement Design Standards ( PIDS)
Manual.
The updated site plan is included in this submittal packet which shows the temporary
emergency turnaround encumbering lots 12 and 15 and meets City standard drawing
for a 21’ wide street in the PIDS manual. Therefore this condition has been met.
4. On the final plat, the applicant shall create an access restriction line along the
westerly boundary line of Lots 1- 3 abutting Hawkins Lane.
The plat creates an access restriction along the West boundary of lots 1-3, abutting
Hawkins Lane. Therefore this condition has been met.

5. A reserve strip shall be conveyed to the City on a standard City form and
recorded concurrent with the final plat. The reserve strip shall be noted on the
final plat as " 1- Foot Reserve Strip Lot A."
A reserve strip to be conveyed to the City on a standard form is included with this
submittal packet. Therefore, this condition is met.
6. Approval for the street name of the private street shall be obtained from the
regional road naming group.
A list of names has been submitted to the road naming committee for approval. Once
that approval is received, then the private street will be labeled on the plat per the
approved name and designated as private. Therefore, this condition will be met prior to
plat recording.
7. The private street shall be labeled on the final plat as "[ Street Name] —
Private."
Once the name is approved by the road naming committee, the name will be labeled on
the final plat as the name-private. Therefore, this condition will be met prior to plat
recording.
8. Prior to final plat approval, the applicant shall submit a Private Joint Use
Access and Utility Easement and Maintenance Agreement (JAM). The JAM will
include a description defining the area of the private street and will also identify
permitted improvements, construction ST 25- 03 Junipepi Springs expectations,
rights of usage, and maintenance responsibilities for the facilities in the
street. The JAM will be recorded concurrently with and cross- referenced on the
final plat.
A copy of the draft Private Joint Use Access, Utility and Maintenance agreement is
included with the submittal packet for review. Once the review is completed, the
document will be signed and recorded concurrently with the final plat. Therefore, this
condition will be met concurrent with plat recording.
9. Provisions for a Public Emergency Access Easement within the private street
shall be included in the JAM that creates the private street. Alternatively, the
Public Emergency Access Easement may be dedicated on a standard City form.
The JAM includes a provision to create a public emergency access easement within the
private street (Tract D). Therefore, this condition has been met.
10. Prior to final plat approval, the applicant shall submit an updated site plan
depicting the necessary public easements for all public utilities not located within
a public right- of-way.

The site plan has been updated to depict all necessary public easements outside the
public right of way. A copy of the final site plan is included with this submittal packet.
Therefore, this condition is met.
11. Prior to final plat approval, a PEPI permit shall be issued for the construction
of public improvements. In lieu of a PEPI permit, a bond or other financial
instrument guaranteeing the construction of public improvements shall be
submitted by the applicant and accepted by the City Engineer.
The PEPI plans are currently in review or the second time with Eugene public works.
The applicant intends to provide a performance bond for the full amount of the PEPI
work prior to recording the plat. Therefore, this condition will be met.
12. The applicant shall submit a street tree agreement application with a street
tree plan to the City Urban Forester. The applicant will be required to submit a
financial guarantee ensuring street trees will be planted and maintained in
accordance with applicable EC 7. 280 requirements. The applicant shall obtain an
approved street tree agreement prior to final plat approval.
The applicant is working with City Urban Forestry on a street tree agreement utilizing
the “city plant” option. The fee will be paid prior to final plat recording so no bond will
be required. Therefore, this condition will be met.
13. The applicant shall provide documentation that EWEB has received a financial
guarantee, ensuring water will be provided to each lot.
The applicant is working with EWEB to obtain the required bonding amount and is
aware the financial guarantee is required prior to recording the plat. Therefore, this
condition will be met.
14. The applicant shall submit an Irrevocable Petition for public improvements in
Hawkins Lane to include street lights.
An irrevocable petition for public improvements in Hawkins Lane for street lights has
been prepared and is included with this submittal packet. Therefore, this condition has
been met.
15. The applicant shall submit an Irrevocable Petition for public improvements in
McKinley Street to include street trees and street lights.
An irrevocable petition for public improvements in McKinley Street for street lights and
street trees has been prepared and is included with this submittal packet. Therefore,
this condition has been met.
16. The applicant shall submit an updated site plan that depicts improvements in
McKinley Street in compliance with EC 9. 6505( 3).

The updated site plan included with this submittal packet depicts the improvements in
McKinley Street to comply with Eugene code. Therefore, this condition has been met.
17. PEPI plans for the proposed development shall include improvements in
McKinley Street that comply with EC 9. 6505( 3).
The PEPI plans currently under review at Eugene public works includes the required
improvements in McKinley Street. Therefore, this condition has been met.
18. The proposed Joint Use Access Easements, which are subject to City review
and approval prior to recording with the final plat, shall be cross- referenced on
the plat and be accompanied by a maintenance agreement of the shared
driveway, which addresses the rights and responsibilities of each parcel relative
to the shared driveway.
The joint use access easements are shown on sheet 4 on the final plat. The
accompanying maintenance agreements are separate documents included in this
submittal packet and are cross referenced on the plat. Therefore, this condition has
been met.
19. The applicant shall submit an updated site plan that shows the Vision
Clearance Area at the intersection of West 18th Avenue and Hawkins Lane.
The updated site plan included in this submittal packet shows the vision clearance area
at the intersection of W. 18th Ave and Hawkins Ln. Therefore, this condition has been
met.
20. The PEPI plans shall include the design details of the stormwater facility
within Lot A.
There is a landscape design included in the PEPI plan currently in review with Eugene
public works. Therefore, this condition has been met.
21. The applicant shall record a deed restriction holding Lots 55, 56, and 57 under
common ownership as a single development site.
A deed restriction related to lots 55, 56 and 57 is included with this submittal packet.
Therefore, this condition has been met.
22. Prior to final plat approval, the applicant shall provide evidence that the
existing accessory buildings on Tax Lots 01501, 01601, and 01700 have been
either removed or relocated to the portion of the development site containing the
church use. Alternatively, the applicant shall record a deed restriction holding all
Subdivision lots under common ST 25- 03 Junipepi Springs Findings & Decision
of the Planning Director I April 2026 Page 48 of 51 ownership until the accessory

building are removed, relocated, or otherwise demonstrate compliance with
applicable standards in the land use code.
The modular buildings on tax lot 1700 have been moved and there are photos included
with this submittal showing they are no longer there. The barn along Hawkins and the
two small sheds on tax lots 1501 & 1601 have not been removed so a deed restriction
for those is included with this submittal. Therefore, this condition has been met.
23. The applicant shall confirm and provide evidence whether there are any "
significant trees" on the subject propepity located with the " brush/ tree line", as
shown on the " Existing Conditions" plan sheet, that were not identified on the
Tentative Tree Preservation and Removal Plan. If there are any additional
significant trees, the applicant shall provide an updated plan that identifies those
trees and includes the required information as listed in EC 9. 6885( 2)( e) 1. a.
through b.
A table, organized by the location categories listed in Table 9.6885(2)(e), listing all significant
trees on the development site and including the following information for each listed tree:
(1) Diameter Breast Height (d.b.h.)
(2) Preservation, removal, or mitigation status
(3) Common name, genus and species
The tree preservation plan that was submitted with the tentative land use packet is being
updated to include two trees that meet the significant definition to be removed as part of the
development in the area that was identified as “brush/tree line”. The trees to be removed
are:
30” Willow
20” Deciduous
The revised tree preservation plan is being prepared by DLA and will be submitted separately.
b. A site plan that includes the following information:
(1) The locations of all significant trees on the development site, the Diameter Breast Height
(d.b.h.) for each significant tree, whether each significant tree is to be preserved, removed, or
mitigated according to EC 9.6885(2)(e)2., and the location of the critical root zone (CRZ) for
each significant tree to be preserved.
DLA is revising the tree preservation plan with this information which will be submitted
separately.

(2) The location of all existing and/or proposed public and private utility easements, driveways,
and areas of grading or excavation on the development site.
The existing and proposed public and private easements, driveway, and areas of grading or
excavation will be shown on PEPI, grading and private building permit plan sets.
(3) The location of all existing development on the site as well as the location of development
proposed in the land use application that triggers the requirement for a Tree Preservation and
Removal Plan.
The final site plan shows the location of proposed development on this site, which is included
with this submittal packet.
(4) Proposed lot or parcel boundaries.
The proposed parcel boundaries are shown on the final plat included with this submittal.
(5) For development sites with any portion located within the South Hills Area, identification of
areas at or above 500 feet elevation and areas at or above 900 feet elevation.
No portion of this site is above the 500 foot elevation, so this is not applicable.
