SE <br />Salem, OR 97317 <br />Subject: EUGENE, OR Vacate (Willow Creek Rd & Slocum Dr) APN 18-04-04-12. Public Utility <br />Easement Vacate. See Exhibit "A". <br />To Whom It May Concern, <br />This letter is in response to the notice for the above referenced public utility easement vacate included in <br />the area. Please be advised that Qwest Corporation d/b/a CenturyLink QC currently does not show <br />facilities in the area addressed by this action. <br />At this time, Qwest Corporation d/b/a CenturyLink QC approves with conditions to the proposed Public <br />Utility Easement vacate request. If facilities are found Qwest Corporation d/b/a CenturyLink QC <br />requests that provisions are made to retain our rights by means of explicit language granting to Qwest <br />Corporation d/b/a CenturyLink QC and its successors" rights that will cover our existing & future <br />facilities. <br />The proposal for a PUE Vacate is submitted with the status of approved with conditions. If there are any <br />changes to the area or terms of the PUE, CenturyLink requests that the applicant provide blue staking and <br />pot holes to locate any possible facilities and relocate the facilities at applicant's expense and within <br />guidelines set by CenturyLink and all regulating entities. All relocations will be done under the <br />supervision of a CenturyLink Inspector. If relocation of facilities is necessary, contact Luke Pilon at <br />luke.pilon@lumen.com.
