THE CENTER FOR MUNICIPAL SOLUTIONS <br />WIRELESS TELECOMMUNICATIONS FACILITY APPLICATION FORMAT <br />While the format set forth below is not required under the ordinance, and the applicant is not legally <br />required to follow it, the following instructions are intended primarily to clarify what is needed for <br />some specific ordinance items in an application and to mitigate the applicant's costs related to the <br />review and analysis of the application. This is the first opportunity the applicant will have to control <br />application-review related costs. Another such cost-control opportunity is for the application to <br />contain all required and needed information, both as set forth in the ordinance and as discussed at <br />the pre-application meeting. This is intended to prevent the consultant having to chase or track <br />down such information. The more complete the application, the easier and faster it can be reviewed <br />and analyzed. Note that the applicant should already have the vast majority of the information <br />required, i.e. for its own business purposes, so there should be minimal original work required to <br />file a complete application. <br />1. Binder: To minimize the time needed to review the information, and thus reduce the cost to <br />the applicant, we request applications to be contained in a 3 ring binder to permit removing <br />and comparing pages, e.g. propagation studies. The application should also contain labeled <br />tab dividers as appropriate for each Section and requirement of the Ordinance, in the <br />sequence set forth in the ordinance. <br />2. Table of Contents: To try to minimize the cost of review and analysis that is born by the <br />applicant vis-a-vis preventing having to hunt for information, applications should contain a <br />Table of Contents showing where each Section of the Ordinance and the response or <br />information related to that issue or requirement can be found. <br />3. Need/Reason: To enable the application to be read, reviewed and analyzed in the context <br />of the applicant's specific needs, and to be sensitive and responsive to those needs, the <br />application shall contain a narrative describing exactly why the new site, colocation or the <br />modification is needed and what is intended to be accomplished, e.g. for reasons of i) a gap <br />in service; ii) inadequate capacity of an existing facility; etc. This narrative should <br />immediately follow the Table of Contents. <br />4. Requests for Relief: Requests for relief must be individually identified and <br />discussed /justified in the application in sufficient detail to understand the need and <br />justification for the request for relief. <br />--~j- 5. Propagation Studies: Required with the application to prove need on any new tower site <br />(proposed height and 10' lower unless otherwise agreed), new co-location on any existing <br />structure, and any modification where there is an increase in the height of the antenna(s) on <br />the structure. <br />Propagation studies are to include a legend on each sheet showing the following, i.e. at a <br />minimum: 1) attachment height of antenna; 2) ERP stated in terms of watts; 3) direction of <br />maximum lobes (azimuth orientation); 4) a scale of miles or distance; and 5) color coded of <br />signal strength designations. Propagation studies are to be provided on a clear acetate <br />sheet so that they may be read individually or in conjunction with each other, as may be <br />necessary for proper analysis.
