the following information; additional application material is required, as <br />specified in paragraph (c) below, for applications requiring a site review <br />or conditional use process: <br />1. A description of the proposed tower location, design and height. <br />Response: AT&T proposes installation of a 75-foot monopole designed to look like an evergreen <br />tree (‘monopine’) located in an R-1 Low Density Residential zone at 4060 West Amazon Drive. All <br />associated equipment will be located in a proposed equipment shed designed to match the <br />existing structure. Details of the design are shown in Attachment 04: Site Plan. <br />2. The general capacity of the tower in terms of the number and type of <br />antennas it is designed to accommodate. <br />Response: The proposed monopineis designed to accommodate at leasttwo carriers. The design <br />includes 12 AT&T antennasat a tip height of70 feet. Future collocation below AT&T’s antennas <br />will be possible at this site, depending on a future tenant’s technology and coverage needs. <br />Details of thisdesign are shownin Attachment 04: Site Plan, page A-3.0 and A-3.1. <br />3. Documentation demonstrating compliance with non-ionizing <br />electromagnetic radiation (NIER) emissions standards as set forth by <br />the Federal Communications Commission (FCC). <br />Response: The proposed site complies with the FCC’s NIER emissions standards. See Attachment <br />06: NIER Report. <br />4. A signed agreement, as supplied by the city, stating that the applicant <br />will allow collocation with other users, provided all safety, structural, <br />and technological requirements are met. This agreement shall also <br />state that any future owners or operators will allow collocation on the <br />tower. <br />Response: AT&T will sign the provided agreement or it may be made a condition of approval. <br />5. Documentation that the ancillary facilities will not produce sound levels <br />in excess of those standards specified in subsection (7) of this section, or <br />designs showing how the sound is to be effectively muffled and reduced <br />pursuant to those standards. <br />Response: The proposed ancillary facility will not produce noise greater than 45 dBA, which is the <br />maximum noise level allowed on property zoned R-1. See Attachment 07: Sound Study. <br />6.
